The Unstoppable Forces Changing Dentistry and Dental Practice

There are numerous strong forces reshaping dentistry and the delivery of dental care. These forces are hurricane-like in power, growing in intensity, forcefully exerting their impact on reshaping dentistry. The eye of the hurricane is the profound shift in context, from small business to big business. The eye-wall of the hurricane consists of the tremendous influx of venture capital and consolidators. The rain-bands of the hurricane are horizontal integration, vertical integration, artificial intelligence, robotics, cloud computing, Big Data, and genomics. Other rain-bands of the hurricane are lack of access, connection to systemic diseases, ashift in reimbursement from the fee-for-service model to the pay-for-performance model, all driven by the seemingly uncontrollable increase in spending on healthcare combined with increasing populations.

This presentation will address how these forces impact dentistry and dental delivery. This hurricane has reached a Level 4 intensity, quickly moving to a Level 5. It has reached the shore, bringing heavy winds and rain. Expect tremendous damage to occur.

Biography:
Dr. Marc B Cooper professional career includes private practice Periodontics, academician, researcher, teacher, practice management consultant, corporate consultant, trainer, seminar director, board director, author, entrepreneur, and inventor. In his career, Dr. Cooper has worked with Silicon Valley start-ups, hospitals, and hospital systems, insurance companies, biotechnical companies, insurance companies, as well as several Fortune 500 companies. Dr. Cooper has worked with dental clients in the U.S., U.K., Australia, Cambodia, Canada, Chile, Portugal, Italy, Greece, Dubai, Abu Dhabi, Oman, Brazil, Singapore, New Zealand and Israel. Dr. Cooperʼs current work focuses on consulting managed group practice in North America and the Far East.

Reasons and Management of Failure of an Initial Endodontic Treatment

In the private practice as well as in the endodontic departments of the dental schools, pain and inconvenience immediately after an endodontic treatment is common. In almost all cases we are able to help our patients through this short annoying period. What happens if, after an apparently successful endodontic treatment without any inconvenience, several weeks later, the pain comes back, just as it was before the treatment? This lecture will give you as clinician the tools to retrieve the cause of the failing endodontic treatment and will help you to manage it.

Biography:
Andreas Braun graduated in 1990 at ACTA (Academic Centre for Dentistry Amsterdam). Andreas is born in Germany, where he finished his school. For his dental study he came to the Netherlands.
From his graduation on until 1999 he worked part-time in a general dental practice and as lecturer at the Policlinic at ACTA. He followed the postgraduate training Endodontology at ACTA (1999 – 2002). Since his graduation from the postgraduate program he works in a private practice limited to Endodontics and at the department of Endodontology at ACTA. He is currently senior lecturer at ACTA participating in the under- and postgraduate programs in Endodontology.
In addition to the general Endodontology, his primary interests are Dental Traumatology and their application in aesthetic dentistry (orthodontic and restorative possibilities), diagnostics and treatment of (severe) endodontic problems and the repair or correction of endodontic failures.
Furthermore, he has written several articles in Dutch and English papers and is co-author of the book: Endodontologie (Red. Thoden van Velzen&Wesselink), 2010. As lecturer he provides theoretical and practical courses on Endodontology inside and outside The Netherlands.

4D Concept and Immediate Implant Placement

The 4th additional axis of time is added to the traditional 3D axes which makes it different to identify the time of extraction and implant placement either immediate implant placement, early implant placement or delayed implant placement.

This review focus on each technique and the ideal choice of each technique in every case

Introduction: Immediate implant placement after extraction has become a favored treatment protocol with many clinicians worldwide. There are many advantages to this protocol, amongst them; shortened treatment time, placement of the implant in sound bone that constitutes the socket wall, placement trajectory guidance by the socket and preservation of bone volume. This literature review describes the 4th dimension in implant placement which is the timing of placement after extraction which is a very important factor in immediate placement success rate.

2. The proper way achieve an perfect immediate implant
3. Decide whether to place or not to place the implant after extraction

Biography:
Ahmed Halim Ayoub completed his DDS, Msc and Visiting Lecturer in BPP University Faculty of Dentistry, London, England. His Diploma and Master Degrees are from Seville University, Spain. He developed expertise in bone augmentation and sinus elevation. He has a special interest in bone healing, the role of growth factors and bone morphogenetic proteins in bone growth. Dr Halim has also highly interested in Dental implanted Education and developed his pioneer modules using Live training throughout direct dental implant post graduate training in Egyptian society of oral Implantology, London oral.

All on Four: Immediate Implant Loading Process in the Mandible

Number of completely edentulous patients is increasing due to increased life expectancy, considerably high number of adult and elderly patients still need dental treatment for their edentulism.

Some of those patients cannot tolerate removable complete dental prosthesis and may require more sophisticated techniques

Almost all patients have information about implant supported dental prosthesis to some extent and they prefer to have this kind of dental treatment because of increased retention and stability of artificial teeth. Unfortunately because of financial restrictions a high number of edentulous patients have to postpone implant supported prosthesis option.

Osseo integration becomes an excellent treatment modality for completely edentulous patients. The better understanding of osseointegration has led to the one-stage surgeries and to minimize the period between the implant surgery and the prosthesis placement.

The maxilla requires more implants than the mandible for a fixed prosthesis although four implants are suggested for either dental arch.

In this presentation; the protocol for immediate loading and all on four treatment modality and long term outcome of immediately loaded all on four type dentures will be discussed.

Biography:
Dr. Hasan Alkumru was appointed as an Associate Professor, Division of Restorative Dentistry, Discipline of Prosthodontics at the University of Western Ontario Schulich School of Medicine and Dentistry, effective September 1st, 2014.
Dr. Alkumru received his DDS (1979) in University of Ankara, Faculty of Dentistry and his PhD (1984) in Birmingham University Faculty of Dentistry UK in collaboration with Ankara University. After that he joined to Ankara University, Faculty of Dentistry as an Associate professor until 1998. He became a full time professor in Marmara University, Istanbul and worked as the Head of Department of Prosthodontics and the Vice Dean in between 1999 – 2008.
Dr. Alkumru joined to The University of Toronto in 2009. He was the Head of Department of Prosthodontics, Director of Graduate Prosthodontics Program and Director of Implant Prosthodontics Unit at the University of Toronto, Faculty of Dentistry. He mentored three master thesis related to implant prosthodontics during his service in UofT.
Dr. Alkumru is the course director of Second Year Removable Prosthodontics course and also gives several lectures to the third and fourth year students in Restorative Dentistry Courses along with other administrative roles in UWO.
He published more than 50 articles in international journals, was selected as Keynote Speaker in International Congresses and has trained and mentored graduate students in the area of Implant Prosthodontics and Biomaterials research throughout his career. Dr. Alkumru is a licensed Prosthodontist in Canada maintains an active practice in his office in Toronto, Ontario, dedicated to Prosthodontics, Implant Dentistry, and Esthetics.

Role of Diagnostics in Oral Cancer – Earlier the Better

Oral Squamous Cell Carcinoma (SCC) is known to arise from sustained, stepwise accumulations of mutations resulting in transition of normal mucosa to dysplasia to invasive carcinoma over time. OSCC is reportedly the tenth most common malignancy and across the world more than 300, 000 H&N cancer cases are reported each year and in India alone it is estimated to be more than 8000 new H&N cancer registered every year. Tremendous breakthroughs are achieved in understanding the biology of oral cancer, surgical techniques, precision radiotherapy and chemotherapy with newer drugs, hitherto the 5 year survival rate of patients has not reduced below 50% since past several decades and have not improved over several decades. Improving detection, diagnosis, and treatment of precancerous changes and early asymptomatic cancers is imperative to increase survival and improve functional outcomes for persons at risk to develop oral cancer. Therefore this presentation highlights the present and future noninvasive or minimally invasive techniques with great specificity and sensitivity in detecting early oral cancers and after treatment follows up to detect recurrences.

Biography:
K V Arun Kumar has completed his graduation from Kuvempu University and post graduation from Rajiv Gandhi Health University, Karnataka, INDIA. He has 17 years of academic experience in teaching and performing nearly all spectrums of head and neck surgeries. He has to his credit nearly 65 national and international publications and Invited Keynote/ Guest speaker at specialty national conferences, CME and CDEʼs. He has 3 International Oral presentations to his credit. Avid reviewer for various national specialty journals and life member for state, national dental and specialty associations of repute and served university posts as past Co-coordinator for the post graduate studies and presently chairman antiragging committee in Subharti university, India. Successfully organized ‘Triple ‘Oʼ conference and Master class for the specialty post graduates at the national level and many state level CDEʼs.

Pedicled Buccal Fat Pad in the Treatment of Class II and Class III Gingival Recession Defects of Maxillary Posterior Teeth –A Clinical Study

Background: Buccal fat pad (BFP) is a specialized vascular tissue adequately present in buccal space and is close to the maxillary posterior quadrant. The aim of this clinical study was to evaluate the utility of pedicled buccal fat pad (PBFP) in the treatment of class II and III gingival recession.

Methodology: Ten systemically healthy patients with age ranging from 35-55 years with class II and class III gingival recession in the maxillary molars were selected. Prior to the surgical phase, patients were enrolled in a strict maintenance program including oral hygiene instructions and scaling and root planing. A horizontal incision of 1-1.5cm was made in the buccal sulcus of the maxillary molar region; buccinator muscle was separated bluntly to expose the BFP. The fat was then teased out from its bed and spread to cover defects adequately. It was then secured and sutured without tension. Clinical parameters probing depth(PD), Recession width (RW), Recession length (RL), Keratinized width of gingiva (KGW) were recorded at baseline and at 6 months post-operatively and weekly assessment was done at 1week, 2weeks, 3weeks and after 4 weeks for observations during the postoperative healing.

Results: Treated recession defects healed successfully without any significant post operative complications. Percentage of root coverage average was 89.3%. There was statistically significant decrease in the width and depth of recession.

Conclusion: Pedicled buccal fat showed promising results as the treatment modality in the management of class II and class III gingival recession of maxillary posterior teeth.

Dr. Deepa D has completed her BDS from College of dental sciences, Davangere, Kuvempu University in 1998 and Masters of Dental Surgery in the speciality of Periodontology from Bapuji Dental College and hospital, Davangere, Rajiv Gandhi University of Health sciences, Karnataka, India. She is presently working as Professor in Periodontology at Subharti University, India. She has worked as co-investigator for World Health Organisation research project in 2006-2007 at Bapuji dental college and hospital. She is an active academician and has published 83 papers in national and international journals and has been serving as reviewer in journals of repute. Have 18 scientific presentations at National and 3 at international conferences at Dubai, Kualalumpur, Malaysia. Awarded –Best scientific presentation at the National evidence based dentistry conference, Gujarat, India and also National awards three times for essay competition during 2003, 2008 and 2016 organised by Indian Society of Periodontology. In 2016 she was an Organizing Committee member for the 2nd World Congress on Dental Research, Kuala Lumpur, Malaysia, where she was a keynote speaker. Invited guest speaker at academic events and recently at 42nd National conference of Indian Society of Periodontology held at Kolkata, 24th to 26th November 2017. She is also a life member of reputed speciality organisations of Periodontology and Implantology.
